Correction of Mutex handling in DdeInitializeW for cases other than 1st call. Addition of instance handling to DdeCreateStringHandleA, DdeCreateStringHandleW, RemoveHSZNodes, FreeAndRemoveHSZNodes. 1st cut implementation of DdeUnitialize, move of internal structure defs in from ddeml.h. Removed non-ANSI comment.
